eClinician PDA Adoption Survey
Dear eClinician Project Participant:
You were among a select group of providers who participated in the eClinician Project and therefore received a Personal Digital Assistant (PDA) from the New York City Department of Health and Mental Hygiene (DOHMH) and Clinical Directors Network (CDN). We are now requesting that you answer a survey (the link is provided at the end of this email) that will help us evaluate the success of the project. Your responses will tremendously impact our future strategic initiatives for promoting technology-based interventions in community health centers.
Answering this survey is entirely voluntary and the data will remain completely confidential. We understand that your time is precious. As a token of our appreciation, all the respondents for this survey will be automatically enrolled in our eClinician Survey Raffle.
